Dear Parents,
We would like to take the children to the Forum at Barrow in Furness to watch the Northern Ballet’s Performance of the Ugly Duckling. The show, which is specifically child friendly, lasts approximately 40 minutes and takes place on Tuesday the 19th of June at 2pm. Tickets are £4 for children and £6 for adults. There would also be the cost of £520 for the coaches to transport the children, but half of this will be subsidised by the Friends of School, so we would ask for £3 per child towards transport.
The Company are also coming into school to do workshops with Years 1 and 2 prior to the visit to the Forum.
If any parents wish to accompany us the tickets for adults are £6 each but unfortunately you would need to make your own way there as there is no space available on the coaches. Sorry younger siblings are not permitted.
Please send in £7.00 as soon as possible. If you would like to pay by instalments this would be acceptable provided full payment is made prior to the trip.

I consent to any emergency medical treatment during the course of the visit. Emergency contact number………………………………………………………………………………………The Local Authority’s Employers’ Liability and Public Insurance Policy will apply during education visits in the same way as they do at School during the normal working day. This means that there is insurance cover only where the Authority or the employees are shown to be at fault for injury to staff, pupils or other persons or for damage to their property. Claims would be against the Authority not against a Teacher personally.
